Question, could it possibly be post round music? or a possible song that was added? Since from what I've seen the server only crashes after rounds, or on map change.

Actually I had thought the same thing, but then went to see what songs it happened on and they weren't the same song, so I tossed the idea.

Could the spawns be broken and cause the game to bug and crash the server?

Aresuft that's a common connection problem when the server restarts/crashes while you're connecting.
However, Brass, I believe it is ULX and ULX has had an update that apparently 'fixed' the issue maybe it works now? Did you check?

I will try updating ULX, however, I feel like that's unlikely to be the cause.
