Knives are fucked in some way

Multiple times now I have stabbed someone from behind, directly behind them with +40% mobi and I can't hit them. Spamming left click doesn't attack.
Are knives using the default Gmod hitbox? If so, there shouldn't be dailies or quests. Throwing knives is OK, but those don't count for the backstab quest.
They're flipping buttmunchingly miserable to use. AAAAAAH.

Yea they use default hitreg. Not my custom one.

Is there any way to implement a custom one or to just change the knife itself? If you give it some extra range, it might register hits better, instead of sticking to someone like glue but failing to stabbity stab.
